Survey Question

I am trying to make a questionnaire where some of the questions are multiple choice. And when the answer is selected, I want it to alter the next question. Basically, the next question is dependant on the answer to the previous question. Then, I would like the data from the questionnaire to be exported into an excel spreadsheet. I am not a skilled programmer. I have some programming education, but very little. Is there a plugin that makes this somewhat simple? I would very much appreciate the help.
