Table doesn't show up when using FORM

Was hoping someone could help.
I had this plugin working just fine, was testing things and submitted a record to a database. This plugin worked fine when I had one record, but once I added the second the form won’t show up. Now, even with just one the form doesn’t show up. If I only use table= and col= I can get it to show fine. However, once I put in anything having to do with form= it stops showing up.
It shows the code in the page source, but there’s nothing there. Really a bizarre issue but I was wondering if anyone has a reason for this.
