It’s good to hear from you – sounds like you’re on a more recent version of WordPress and having good results with MLA.
I suspect that the WordPress Tag Cloud widget only works for posts and pages. I will have a look at the code and see if that’s true or if there is some other issue specific to attachments. I will post an update here and let you know what I find out.

I had a look at the WordPress code and confirmed that the standard Tag Cloud widget will only display posts that are “tagged” with a particular term.
There are several “tag cloud” plugins and widgets in the repository; I have not investigated them. You might find something that does what you want.
I might be able to enhance Media Library Assistant in some way that meets your needs, but I’d like more details about what you’d like to see. With blog posts, the tag cloud takes you to a page displaying the posts that contain the tag. What should happen for Media Library items? I imagine a page that contains a gallery listing of the items that contain the tag. Is that what you have in mind? If so, how would you specify which page to display?

A late response – I’m using “Better Tag Cloud” for this. It’s a bit old but still works fine for me.

I have released v1.60, which includes the new [mla_tag_cloud] shortcode. I believe this new shortcode will satisfy your requirements.
