Taxonomy Permalinks

Hi

I am trying to set up a site and this will be the first time i have used custom taxonomies, i want the url to be like the following

/%placename%/custom-Taxonomy%/%postname%/

The above works and so does this

/%placename%/

but this just 404’s

/%placename%/custom-Taxonomy%/

I have tried flushing my rewrite rules many times, disabled all plugins tried different themes and fiddled with all the custom post type settings and custom taxonomy settings but still no luck!

Im pulling my hair out not, any help would be appreciated.

Just to confirm also non of the custom taxonomy names conflict with any other page names.

Im happy to post any code you need to see.
