I think that could be done with our Notification : Conditionals extension. You could create a different notification and control the submission based on the trigger criteria.

What do you mean by model? The email itself is built one and is meant to be sent automatically, without any action from the administrator to process it.
We currently don’t offer to configure exchangeable templates, only a static notification. The Email Templates plugin we’re working on could be helpful, but this needs a few more months until released.
