    The form does not show up on the website version 1.5.01. I delete Install the version from your website 1.4.28. The form should show on the right side of the page, beside the truck [ redundant link removed ]

    The page I need help with: [log in to see the link]

    Your website is using an outdated commercial version of the plugin. Update to the latest commercial version to correct that problem.
