Which version of WordPress, Go and CoBlocks are you using?
I just tried to fill both of the forms you have on this page and it says that the messages were sent. If you do not receive these in the next minutes it seems to indeed be a bug.

We indeed made some changes to the Form in 2.21.5
It was working with 2.21.4 and/or before?
This is odd. I replaced the original page form with a new form and it appears to be working now.

So the original page form didn’t but I added a second form and it worked, then I deleted the original and kept the second added form and it appears to work now.
Ok. Happy that it now works! Feel free to come back here if you have more issues!
