It seems to workform my side. When the submit button doesn’t work in most cases the cause is a transparent layer located over the button. In those cases adding a <div style=”clear:both”></div> after the shortcode or placing the shortcode into a leyer with higher z-index will avoid the transparent layer. However in your page I didn’t experience problems when using the submit button.
If you are still getting the problem then open a ticket at form2email.dwbooster.com/contact-us and indicate there the browser and device you are using when experiencing that problem.

Thank you for your prompt response, i added your suggested code ans still have the same problem. it seems the button is not working and it is not going to the next step. since yesterday that i lunched the website i had 4 customers emailing me that my questionnaire is not working. could you please help me. I don’t have any idea what the z-index is too …
