The form example you sent along is pretty heavily customized, and would take quite a bit of custom coding to get that done. I do have some recommendations for you, but before I jump to those, I want to share that GiveWP is deep into work on version 3.0, the next very big release for the plugin. This release is specifically changing the way donation forms are created and styled, so it’s important that you are aware of that, especially because you are looking at custom coding. If you’d like to move ahead with customizing a form right now, I’d recommend using the Legacy form template. Classic and Multi-Step templates are served in iframes, which makes them very resistant to customization, and that is by design. You’ll have the best luck with Legacy, and then using our CSS tutorial found here: https://givewp.com/documentation/resources/how-to-style-your-givewp-forms/. You’ll need to use CSS and custom coding for the recurring options as well.
