How are you ouputting that form exactly? Via the shortcode? Or a text widget?
It looks like WordPress’s wp_auto_p function is adding line breaks in unnatural places which is causing that misalignment.
Clarify how it’s being output for me and we can see from there. Thanks!

Also, I uploaded the Authorize.net addon and I am not seeing anywhere to add the key where it says it should be
Go to “Donations > Settings” and click on the Payment Gateways tab. There you’ll see a section called Authorize.net Gateway Settings.
It isn’t there for me.

Sorry for the delay. Please try going to “Donations > Settings” then the Advanced tab and check the box next to “Disable the_content filter”

I am currently finding the same thing with my forms. The First Name and Last name are not lining up. There appears to be a line break between the two. Is there a way to correct this?
