Not at all sure – I can see you’ve got the alt attributes working, which does show the mouse over, but the title tag isn’t there.
Could it be something your theme does when displaying images?
My admin went ahead and installed “SEO Friendly Images” http://www.prelovac.com/vladimir/wordpress-plugins/seo-friendly-images
But this is taking the Post title and using it as image title.
If you want I can deactivate it to demonstrate your plugin. I am using StudioPress themes (Genesis framework)

Do you get the Title tag? The Alt tag can be seen in the HTML source.
Please guide on what should be done to get Title tag to work.
The title tag is definitely not there.
The plugin doesn’t do very much at all, so it’s difficult to see why it wouldn’t be working.
When you upload an image to your site, and before inserting it, is the “title” field completed? It should look like the screenshot:

Do I need to activate any debugging in your plugin, I would be happy to look into it
