toc not working

initially toc worked on a long post. but then when I made some changes and removed some headings the toc wouldn’t refresh. I also had no success with the widget toc+. It just doesn’t seem to recognise it.
I am using the weaver11 theme and have had plugin autopaginate and wp-pagenavi installed at one stage. I have deactivated them but still problem seems to continue.
