Trouble with 404 permalink fix…?

Hi,
I’ve got wordpress up and running:

http://blog.surelock.org

It’s installed on a Windows, shared hosting account – so had to use this method for the url rewriting (permalinks)

The default page is broken…
Every other page is fine, however the index page (front of the blog) doesnt work.

Any ideas why??
