Trouble with form designer shortcode

I am sure this is me being dense, but it seems like I cannot do anything with the shortcode, except embed a form, no modifications happen.

I would like the form on this page:

to have an email address box (which is does), a note to say sign up for these lists, and the three lists with checkboxes. This is what I am using, with no success.

[constantcontactapi formid="1" list_selection_title="Add me to these lists" lists="38,45,46"]

Any suggestions where I am going wrong?
