Just to be sure, in the Form editor under “Form placement”, you don’t have it enabled in multiple sections (i.e. “Fixed bar” AND “Below pages”), correct?
As you mention, that page builder could certainly be affecting things. Please try temporarily disabling Bricks Builder and see if that resolves the issue. To avoid affecting your customers, you may want to do this on a test/staging site. If you don’t have one already, your hosting company might offer one as part of your package. Alternatively, you can use a free plugin like https://ww.wp.xz.cn/plugins/wp-staging/

Unfortunately, Bricks Builder doesn’t work that way, it’s the theme and builder in one, so it cannot be disabled.

In this case, you can create a staging site as Kelly suggested and switch to a default theme such as Twenty Twenty-One theme.
