Two Forms On One Page Issue

I have to forms on a page http://www.essentialinsurance.co.uk/income-protection/

When using ipad / iphone and submitting the callback request form it sends the message but validates the form in the footer at the same and brings up validation errors on that form.

Any help would be appreciated!
