ugly permalinks

I’ve installed your plugin and assigned a link to click through to so I can display a single entry of the Directory.

When I click the link, I get something like this in the address bar:
http://www.mywebsite.com/member-directory/entry/3/13/

This is not desireable.

The single display also is ugly. Instead of the post title, I get this on the single view: Getting to Know You : Entry # 13

Please let me know what settings I need to change to fix both problems. I’ve been banging my head on this for several hours and I’m out of ideas.
