Unable to save plugin settings

    When attempting to save this plugin’s settings, I get:

    Notice: Undefined index: send_welcome_html in /.../wp-content/plugins/dm-confirm-email/models/plugin-page.php on line 31

    I had unchecked the “Send as HTML?” checkbox, however the “Send Welcome Message?” checkbox was still checked.
