    Hi,

    I tried to create a survey form and when I’m trying to submit the form, there’s an error occur. How to solve this? Please see the error details.

    Hi!

    Could you please check the permissions for /wp-content/uploads folder and /form-maker folder?

    Also, please make sure that there is no problem with including Javascript files in .htaccess.
