Undefined Variables on line 621

I am getting undefined variables for all variables ($success, $error, $gct_name, etc). The submission form still appears under all the undefined errors. This happened when wordpress went to 4.0. I appear to be the only one experiencing this when I searched for a cause. I tried defining all the variables at the start of the script as NULL but this did not work. Is there something wacky with my site?
