    I keep getting a failed login and a successful login message in the audit log and also an email sent when I successfully login to my site.

    I logged in successfully and did not enter a wrong password or login credentials. Why am I getting the failed login message?

    I get this first : Unknown User authentication failed: Unknown IP: 123.345.345.666

    Then I get this : mcam User authentication succeeded: mcam IP: 123.345.345.666

    Unfortunately, I am unable to answer your question right now.

    When the plugin reports a failed or successful login, it does so because WordPress triggered the "wp_login" or "wp_login_failed" action respectively. I just tried to reproduce the issue in my server and it doesn’t happens the same why as in your website, there must be something else going on in your environment that is causing the trigger to run, notice that it also says "Unknown" where the plugin should report your username, meaning that WordPress is actually missing the metadata for the account when passing the login attempt to the plugin.

    I will pass this ticket to my project manager and they will assign a priority. I will continue investigating when they pass the ticket back to me and will notify you when we have a solution for the error or an explanation to justify why it happens.

    I think I found the issue and I’m not sure if it is the plugin or Sucuri.

    The plugin that is causing it is: Login No Captcha reCAPTCHA

    When I access the wp-login page Sucuri send the message :

    Unknown User authentication failed: Unknown

    whenever I access the page and do not enter any login or password. After login and password entry, I do receive a successful login message.
