I don’t see form outside which version of Forminator are you currently using but I can see the form on homepage loading just fine.

Is it with newest Forminator or did you downgrade?

Have tried t simply clear all cache on site/server after upgrade? Was only Forminator upgraded or some other plugins or theme at the same time?

Finally: what are WordPress and PHP versions powering your site?

Please also inform us about your PHP version that you can find on your WP Dashborad->Tools->Site Health->Info->Server->PHP version section.

Then after updating to the latest version again, please try enabling “Load form using AJAX” and “Prevent page caching on form pages” options under Edit Form->Behavior->Rendering

If that does not help, then I suggest to perform full plugin/theme conflict test.

Please make sure you have recent backups and perform this operation on your dev/staging site if possible.
