    Hi,
    I am not sure how you putting default values to your input fields on PR form Editor. However, you will need to set Redirect Option on “Submit Button” at Form Editor. You can expand Submit options by clicking on pencil icon on the button. Choose appropriate redirect option and select either page or put custom URL for thank you page.

    Regarding your other question, you want default look of registration form, you can still use:
    http://hardbodytraining.net/wp-login.php?action=register

    Just make sure you set the option to add PR fields on default Registration screen at PR General Settings page.
