I have a very old blog that is currently using version 2.0 of WordPress. What is the most direct upgrade path from that to the most current version?

I would upgrade it to 3.4 each time skipping two versions. 2.0 to 2.2 to 2.4 etc..

Disable all plugins and switch to the default theme.
Create a full backup first and make a backup of your database after every upgrade. Check after each upgrade if your site is working correctly.
