Upload file names corrupted

After uploading a PDF, the record is being corrupted with the following showing in the top meta box (Upload File from my PC) on the edit screen:

ol> --> mydocument.pdf
KB

This in turn breaks the entire plugin and all my uploaded files fail to list in the File Manager list view. I checked the database and sure enough the field “file” shows only this:

             <!--            <li> () <a href=\"\"></a></li> -->
             <!--</ol> -->
mydocument.pdf

Why is all that html commenting, lists and garbage attaching itself to the record and showing up? What would be causing this?
