Uploading Media/Permalinks

Hi,
I am having an issue uploading media to my site. I originally had my site at http://npinternalmed.com/wordpress_1564113898/ when I first uploaded all my assets. Now I have changed the root URL to just http://npinternalmed.com.

However, now I receive a 404 error anytime I try and upload media. If I hardcode the /wordpress_1564113898/ back into the URL of the newly uploaded asset, I do not have a 404 error. I tried troubleshooting with the permalinks settings but I haven’t had any luck so far. I would like all media uploaded to default to my new URL.
