Is it possible for the portfolio grid item to point to a custom url rather than the actual portfolio item?

You can use custom images set instead of portfolio items.
If you don’t want to, you can try to set redirect URL to your portfolio items using a 3rd-party plugin. Something like this one https://ww.wp.xz.cn/plugins/page-links-to/
