    find all posts and pages with no featured image and insert a default. It doesn’t do that no matter what combination of options. Only some get the image and most are not updated. I don’t want to overwrite existing images.

    Here is what this looks like.

    So either your instructions are misleading or the plugin won’t add images to posts without a featured image already, which is what I expected it to do.

    Oh, you can do that! There is not yet an extra option to consider only “featured image-less” posts. But the plugin has the option to keep existing images. So the usage is:

    1. Make a backup of the database, please!
    2. Select the default image
    3. Make sure the “Overwrite” option is not checked
    4. Use filters if neccessary
    5. Preview
    6. Refine if neccessary
    7. Perform

    That is what I did, it disregards all without featured images. This is a dev site, so I can experiment.

    Ok, it is a UI issue. The real way to do it is:

    Replace featured images by the selected image.
    Don’t overwrite
    You will have to click on the checkboxes with no images (confusing because there is no feedback)
    Then you will see the ones that are going to be replaced.

    No, you do not need to use the “checkboxes with no images”. These are the filters you can add. Just run the plugin over all posts. If will not change existing featured images since you have not checked the “overwrite” option.

    Oh, now I saw your screenshot. That is a strange case for me. I can not reconstruct it on my servers. I am just thinking about a reason.
