User Interface[Write testimonials option]

Hi There,

How I can provide a user interface to write a testimonial rather than navigating a user to dashboard.??

Here I do not want to navigate the user to dashboard to write the testimonials. If a user want to write a testimonials he should have get a pop up page(i.e another interface where he/she can write it.)

I am really looking for write testimonials option. As simple as below.
