Using Jetpack Subscription form in Scroll Triggered Box

Hello there, I am using the Scroll Triggered Box plugin by Dreamgrow.. I want to use it as a subscription form for my blog..

I have the Jetpack subscription form code

But i am not able to use it in the Scroll Triggered Box as it is looking for the “form action” which in this case is #..

Can you please guide me on what form code(s) i must use to make the Scroll Triggered Box work as a subscription form using Jetpack..

Or if Jetpack has something like the Scroll Triggered Box, please let me know.. Thanks..
