Validation Section Not Working on Old Forms After Plugin Update

Hi,
I recently upgraded to the latest version of Visual Form Builder and immediately received a number of warnings on my pre-existing forms. I went into each of the pre-existing forms and clicked the “save” button again, thinking it would clear up the errors – and it did.
However, now I’m noticing that I can no longer press “submit” on any of these forms. As a matter of fact, the entire “verification” fieldset doesn’t ever appear in my editor!

http://forms.mta.org/sponsorship-questionnaire/Please note, that I HAVE created new forms from scratch and they work without a problem. I also switched to the default WP theme to verify that the problem still exists and is not in the theme.
