Variable product adding all options to cart

I have a grouped product with a variable procuct inclluded. Somethines it will allow the buyer to choose one of the variables and add it correctly. Other times when they choose it adds both variables to the cart. I have disabled all plugins and tries with same results. I find no cached active.
