I’m sorry this is happening to you. I just tested this functionality locally, I couldn’t replicate this issue locally. Could you temporarily disable all other add-ons except Ninja Forms and any Ninja Forms add-ons that you have and switch to a default WordPress theme like TwentySixteen?

Also, many email hosts will reject emails sent “To” and “From” the same address. Can you make sure that you are using different email addresses in the “To” and “From” fields in your Email notifications.
As a test, can you try a service like MailGun to manage your email? http://wpninjas.com/sending-email-with-mailgun/
