Video widget disappeared

I downloaded video sidebar widgets, then successfully added a normal video sidebar widget. However, when I came into the office this morning I found that the widget had disappeared both from the sidebar and the available widgets menu. Yet, both the random and meta video widgets were still available in the widgets menu.

I deleted and reinstalled but for some reason the standard video widget hasn’t come back, though the other two have.

Any help would be greatly appreciated.
