This is exactly the same issue that I was having recently.
It transpired that the issue for me was that I was using CloudFlare and it was caching the webp version of the images. When users of the website visited they were served with the CF webp version – fine for most but not for the iPad/Safari users. In the end I gave up and switched off the webp option in LSC.
However, I’ve recently (after upgrading LSC to v3) started to use QUIC.cloud. I’ve re-enabled webp in LSC and everything now seems to be working as it should for iPad users. 🙂
So, in my experience anyway, the LSC plugin isn’t ‘coded wrong’. It was a result of the way that CF worked.
Are you using CF? Or another CDN?
I do not use cdn, QUIC.cloud is expensive?
If you aren’t using CF then I’m afraid that you may have a different issue to the one that I had.
Anyway, QUIC.cloud is free at lower levels (I only have a small blog). Why not take a look (you can always ditch it if you don’t like it)?

If you have object cache , please try turn it off , purge all , and see how it goes.
And also , please provide the report number , you can get it from toolbox —> report —> click “send to LiteSpeed”

this bug on other page actually
