There was a JS error in /wp-admin/load-scripts.php that prevented Save All Settings buttons to display correctly.

If you experience similar problem try to replace /ad-inserter/js/ad-inserter.js with the file from trunk:
http://plugins.svn.ww.wp.xz.cn/ad-inserter/trunk/js/ad-inserter.js
