I can see that the CSS and JS files for the booking form are getting loaded, so you’re probably running into the same issue. Can you list step-by-step all the actions you’ve taken to try to get this working with the page builder?
I couldn’t solved with that solution. Why is restricted to one form in a page?
I’ve solved putting with php in shortcode. In page builder is not working… 😉
