Please share the name of Activated Theme, quite possible the themes uses Ajax to update cart , not html directly which can be cached and will not show real time changes.

Please confirm theme use AJAX to display cart update value.
Flatsome Theme has resolved an issue where the header cart item element was not updating in some cases. The issue has been fixed in current version of Theme.
Please update latest version of Flatsome Theme.
