Thanks for the suggestion SelfOutlet. Gave it a try but my issue still persists.
There was an orders page problem with WC v3.5.2 when WP v5.0.2 came out. WC was fixed with v3.5.3, do you have that latest WC version?
Yep, I have WC v3.5.3. Ive also tried using the plugin Admin Columns to force the columns to show, but no changes. I have plugins for pdf invoices and shipping tracking and those are the only two columns showing.

@cipherskincare – you mentioned that you tried disabling all plugins except for WooCommerce and switching to the default Storefront theme. Once you did that, did you try creating a new order?
If not, please try creating a new order with your plugins and theme disabled and check again.
If the order information shows up properly, it means that either your theme or a plugin is impacting the order information that is being saved to your database. At that point you can reactivate your theme and plugins one at a time, as you make test orders to help find the cause of the problem.

Thanks for the suggestion as I had not tried it yet. Still no change. With all plugins disabled and storefront theme the order details table is still without columns when I submit a new order. I actually couldn’t open the order details in web view at all since there were no plugin populated columns for me to select.

I’ve corrected my issue! Although I’m still not sure what caused it, here’s my fix.
First, I re-installed WordPress (again). The Orders table was still not populated, but I was able to add columns within screen options this time. My bulk edit select all checkbox was still missing, therefore miss-aligning the entire table and messing with functionality, but I was able to use this fix to recover it: “Select All” checkbox header is missing from “Orders” admin panel page #11764
