wordpress 4.1 visual editor issue all themes

Dear Team,

Today we installed wordpress version 4.1 and the visual editor for images on any post or page has stopped working. We can insert an image in the theme but cannot add alt text or change dimensions using image editor inside the post.

Tried using different themes but the issue is there for all themes. Reinstalled the wordpress also couple of times.

We tried clearing browser cache and removed all plugins the issue is still persistant.

Read all threads and now don’t know what to do. So asking for help?
