Does it appear if you refresh the page?

So it appears every time you reload an admin page in Redirection?

Have you ever installed one of the Redirection beta’s?

Version 3.0 does include a change to your database, but it should only run once.

Do you have access to your server’s error log? If so, could you take a look there and see if anything is shown for Redirection?
