WordPress Photo Contest app not loading correctly

I am trying to set up my wordpress, and I want to run a photo contest on the WP PhotoContest app, but when you click enter contest on this page http://lynnylens.com/test

it looks all weird. It’s loading the file and folder directory of the plugin folder and I don’t think it’s supposed to do that. I need this to work but I dont know how to get it to work

Can someone help me?

And sorry about the looks of the site, I just started it today.
