WP FORM CUSTOMIZATION.

I need some customization, how to contact you before buy the plugin? Kindly provide me the email. Is it possible to customize?

I need a survey form with following feature:

1. Admin Should View in wordpress dashboard backend to Sort out Who are the Hot/cold/warm users

2. If people choose this answers will be considered as hot, and their submission will be store in as hot category & also send them auto email with Hot email template.

*Then in weekly or monthly there should auto email reminder to that hot/cold/warm emails with newsletters or anything, if can not do auto, admin should see the recipients and send email from the list in wordpress backend
