WP_CONTENT_DIR issues with Trellis

Trellis sites use /app for everything that is not downloaded and included in the WP.org zip files. The way WP_CONTENT_DIR is set, it will prefix the /app with /wp causing the links to not work. For example we have broken images here in the nav menu https://www.identitymanagementinstitute.org/
