It is the first field “Gift Certificate Amount” – it is a Single Item Payment Field.

I wasn’t able to replicate the issue but i think that the problem is that your single product field has an empty price. The single item field displays the field price in the pdf so since it is empty in the pdf it would be shown as empty as well. Perhaps you want to display the item label instead of its price?
I need the user to be able to enter the price, just like any other field that they would complete. I just did another test with a dropdown item payment field, and that did not work either.
After some testing, I am finding that the issue seems to be if the single item payment field is required. If the field is set as required then it does not display in the PDF. I am now able to make it display if I set the field as not required, however that is not really ideal. Also, the inputs are not formatting how they are set to be – they are not centered and are not formatted as numbers with the dollar sign and 2 decimals, as they have been set in the PDF form.
I just purchased the pro version in hopes it would resolve my issues but it did not. I need this to work with payment fields please. For me, that means…
– The user needs to be able to input the price (the purpose of this is to create a gift certificate so they need to be able to enter the amount) and the field needs to be able to be required
– The payment field data that displays in the PDF needs to be in US currency format ($1,000.00)
– the fields need to be styled as they are set to be styled in the pdf/form (center aligned, bold text for example)
