“Your login confirmation link”

I recently setup WP 2FA on a multisite instance for all sites in my installation. I have a user on one site (many other users can use successfully) who doesn’t get an emailed OTP code but gets a email “Your login confirmation link for XYZ site” with a link. The text of the email is
—Someone from x.x.x.x is trying to log in to the XYZ Website.
Verify the login by clicking this link. If this is not you, please ignore this email and contact your website administrator.
Note: as a security precaution, the login verification link only works when the link is clicked from the same browser and device combination from where you are trying to log in. If this is not the same browser and device, please copy the link and manually paste it in the address bar of the browser which you are using to log in to the website.
—This sometimes works but not sure why its not prompting for the OTP code when I have this set from network admin settings and sometimes its just an infinite loop of this clicking the link and then doing it over and over.
Any ideas?
