Your Session has expired loop

I installed Two Factor, and setup Google Authenticator as the primary method.

It worked great until I tried to log in a few hours later — now it says “Your session has expired because it has been over 60 minutes since your last login.”

I try to log in again, get to the code entry, enter the code, and then it gives me the error above again.

Any idea what might be happening?
