Your WordPress site depends upon PHP to process the .php programmed WordPress core and its plugins. There is a PHP module called “php-calendar” that must be included in your webhosts PHP installation.
If you have access to EasyApache4, then you can add in the required modules yourself. If this sounds unfamiliar, then reach out to your webhost support team.
Here is a typical list of PHP modules included when PHP is provisioned for a webhost:

I too just noticed that adding New Account for Facebook that the Nextscripts API using either UserID and SessionID or username and password is no longer available. Yes, I have a PAID Pro subscription and that API is the big reason for doing that.
No communication from the developer about the use of their Nextscripts API. I never have gotten it to work and continue posting for more than 1 or 2 posts before having to set it up all over again.
What I am asking about would be similar to what is being asked in post, https://ww.wp.xz.cn/support/topic/having-url-parameters-populate-into-form-fields/
I see that the developers are working on this as a feature. I look forward to seeing what is released.

Thank you for your reply.I did not think that version 2.13.25 was that far out of date. The Form Maker plugin was just updated on my WordPress site less than 2 weeks ago. I have gone ahead and updated to 2.13.26.
After updating Form Maker to 2.13.26, I tried your suggestion as outlined in the screenshot you shared. While the UI allows me to enter a WHERE lookup using the parameter {2}, which is my form field “Trip Name”, which matches a field/column in my MySQL table, the form preview (in a separate preview browser window) did not populate with matching info from my selected field, “room_type”. The form choices were blank.
I should mention that my WordPress site is running version 4.9.13. The latest WordPress version available is 5.3.2. I have not yet made the move to 5.x.
Since I am using the Premium version of the plugin, I will follow up via the Support Form as you suggested, rather than the forum.
