i activated it in the wordpress plugin menu.
it’s been running for nearly a year without problems. i used it 2 weeks ago without an issue as well. this is a new bug.
something similar happened to me, but i can’t even access the backend of some of the forms – just getting white pages. worked fine for nearly a year until last week…

Not sure if this helps people to better understand the issue but I think it has something to do with the rather long drop down menu of countries.
Is there a limit to the number of items in the drop down menu?
