You can either upload it to your plugins directory through ftp or go to “Plugins” in the Admin panel and choose “upload” and upload it.
Once uploaded, click on “Activate plugin” that’s it!

In reply to: [WP Feedback & Survey Manager] Code is breaking on pageI have this same issue – first off: great plugin man I like it but I really don’t think it’s a theme issue as it can’t be everyone’s theme causing the error. Second: Yes I think it may possibly be a plugin issue but I have deactivated all plugins and reactivated one at a time to no avail. I am keeping the plugin tho because I only have this issue when I use the feedback tab, I don’t get any error when I only use the “personal info” and “survey” tabs.
Only happens when I include the “feedback” tab and it’s the same exact issue as nirion209 above. If I run across a fix I will post it for other users 🙂
I would think that you could just put the text in the box, or if you wanted to have your name link to your personal profile you could do something
